H1N1 School Clinic Update

Dear Parent/Guardian,
Happy 2010 and wishes for a healthy new year! We want to let you know that the Public Health- Madison and Dane County is planning to resume the H1N1 immunization clinics in the McFarland School District. At this time, clinics are scheduled for the following dates and locations:

WIS February 3 February 8
IMMS February 5 February 10
Due to the lapse in time from the receipt of the original consent forms (October) and the likelihood that some information may have changed, we are requesting that a new consent form be completed and returned to your child’s school health office. You may also download new forms from the McFarland School District’s website in an English version or Spanish version.
Please note that on the Public Health consent form you need to pay close attention to questions 1, 3 and 4. If you check yes for these questions, explain in detail the type of allergy, vaccine, or reaction. This information will assist the Public Health staff in determining type of vaccine for your child-nasal or injection.
If your child has received the first dose of the H1N1 vaccine elsewhere, and needs a second dose (required for children ages 9 or under), he/she may obtain the second dose at this school clinic. However, it must be at least 21 days from the first vaccine to the second dose. Please indicate on the consent form if you are requesting the second H1N1 dose. Also include the date of the first vaccine (dose).
Note: Public Health will not be conducting an additional round of school clinics. For children (age 9 and under) who will require their second dose, please refer to the Public Health website for information on locations of community clinics or contact your health care provider to obtain the second dose.
