- A parent and/or guardian may make a request (referral) to the district for a special education evaluation of their child by contacting the following school personnel:
- Upon making a referral, the evaluation process begins. Within 15 business days an IEP team will be assembled, gather information, and determine the need for additional testing. This team includes a minimum of parent(s)/guardian(s), special education teacher, general education teacher, and a district representative (LEA).
- Information is gathered from parent(s)/guardian(s) and school staff regarding a child’s academic and behavioral performance, social emotions skills, medical diagnosis, etc.
- If additional testing is needed, parent/guardian must sign consent and agree to testing. Upon receiving this form, the IEP team has 60 calendar days to complete and hold an evaluation meeting with the IEP team to determine a child’s eligibility for special education and the need for specially designed instruction in the school environment.
|
- At the meeting, school staff will present the information gathered regarding the child being evaluated from all previously discussed sources. This may be in the form of norm referenced test results, observations, rating scales, or medical reports, etc.
- Once all the information is presented to the team, a determination of eligibility for special education will be determined using the eligibility criteria worksheets for each impairment area of concern.
- If a child is found to be a child with a disability, the IEP team must hold a meeting to develop and implement an Individualized Education Program (IEP) and determine placement for the child within 30 calendar days of the initial evaluation meeting date.
- Re-evaluation for special education takes place every three years.
- If a child is NOT found eligible for special education this concludes the process of evaluation for education. Parent(s)/guardian(s) may refer their child for additional special education evaluations no more than once a year unless the child’s parent(s)/guardian(s) and the district representative/administrator agree otherwise.
|
- Once a child has been found eligible for special education, the IEP team must hold a meeting to develop and implement an Individualized Education Program (IEP) and determine placement.
- This program is tailored to the individual needs of the student including, but not limited to, goals in the area(s) of need, modifications and accommodations necessary to achieve academic and/or behavioral success in the school environment, related service needs, specially designed instruction in the area of need, etc.
- IEPs and placement must be reviewed at least annually.
